Parents or guardians of new students should present the following items at the time of registration:
- Student's Birth Certificate - Acta De Nacimiento
- Student's Immunization Records - Immunizaciones
- Student's Social Security Card - Student's Social Security Card
- Proof of Residency (i.e. mortgage contract, lease agreement, rent receipt) - Prueba De Residencia
- Last Report Card/Withdrawal Form (previous school) - Datos Escolares - Incluyendo Pruebas/Examenes
- Drivers License of Parent or Guardian of Enrolling Student (In accordance with Senate Bill 1432, a school district is required to record the name, address, and date of birth of the person enrolling a child)
Limited Open Enrollment SMSD accepts application for students living outside the SMSD boundaries. Approval of applications is based on review of specific criteria.
